Pilot indecision, a failure to engage thrust reversers and heavy rain were among the factors contributing to a serious runway incident involving a Qantas 747-400 at Bangkok Airport, says an interim report by the Australian Transport Safety Bureau.

The aircraft, with 391 passengers and 19 crew aboard, overshot the runway Sept. 23 and continued for about 80 m before coming to a stop with the nose resting on an airport perimeter road. …
