Spanair MD-82 that crashed Aug. 20 on takeoff from Madrid Barajas may have taken off without its flaps and slats extended, according to the Wall Street Journal, which cited sources close to the accident investigation. Investigators are examining why, if the aircraft was not in takeoff configuration, an automatic audible warning did not sound in the cockpit.
