Geoff Dixon's final few months as chief executive of Qantas have been anything but restful as the airline has confronted a string of operational and safety-related mishaps resulting in a special audit by Australia's Civil Aviation Safety Authority last month.

The most serious was the dramatic and highly publicized decompression event involving a QF 747-400 en route from Hong Kong to Melbourne with 365 passengers and crew on July 25. The aircraft made a successful emergency landing in Manila with no injuries but with a large hole in the fuselage around the wing root area.

All evidence points to an exploding emergency oxygen cylinder in the baggage compartment as the culprit, although the investigation continues. …
